The enrollment form must be on file with the Caughlin Club prior to leaving your child. Additionally, the Billing Information form must be on file if you are not a member at the Caughlin Club.
You can download, print and fill out the forms below and bring them with you to the Caughlin Club if you like. Alternately, you can drop in and fill them out in-person.
